CIS Internship Forms

Students 

Internship Request Form

This form is to be used by current Harper College students only to submit a request for a CIS internship.

Internship Information Release Form

This form will allow Harper College and the CIS Department to post details about the internship such as the student's name, the company's name, the semester of the internship and a general description of the work to be completed.  The testimonials are to be used for recruiting purposes.

Student Internship Weekly Summary Form

This form is to be submitted weekly by the intern summarizing the week's activities.

Student Final Report Form

This report describes the work environment and experience in 2-3 pages. The form is to be completed at the end of the internship

Student Evaluation Form

This form is to be used by the employer to evaluate the intern at the midpoint and end of the internship.  The form is also used by the employer to discuss the intern's performance.   The student is encouraged to look at the form to be aware of how they are being evaluated.
