Student Request for Semester Interpreter
Before you can submit your request, you must read the Consumer No-Show Policy and agree by clicking on the "I Agree" link below.
Consumer No-Show/No-Call Policy
Inform the ADS office as soon as you know you will be absent. ***You must call ADS at 847.925.6266 (voice/video phone) or 847.925.6011 (TTY)***
Guidelines for notifying the ADS offices:
- 7 am classes: contact ADS no later than 6 am to leave a message.
- 8 am classes: contact ADS no later than 7 am to leave a message.
- 9 am or later classes: contact ADS at least 2 hours before start of class(es).
When calling to notify ADS of an absence, provide the following information:
- Your name
- Name of class(es), for example: MTH103
- Class time(s) and day(s), for example: M, 9-9:50 a.m.
- Name of interpreter(s)
- Are there other Deaf/Hard of Hearing students in the class?
The interpreter will wait outside the classroom for ten minutes after the class starts. If you expect to be more than ten minutes late for class, contact the ADS office before class, or else your interpreter may be reassigned.
Dropping a class
Consult with your Counselor, then fill out the Interpreter Request Cancellation form.
If you do not notify the ADS office and are absent for a class
After 2nd No-Show / No-Call per class / per semester:
Interpreting services will be suspended for that class. You must arrange a meeting with the Interpreter Coordinator. After the meeting, interpreting services for that class will resume.
After 3rd No-Show / No-Call per class / per semester:
Interpreting services will be cancelled. Arrange a meeting with the Interpreter Coordinator. Services may resume after interpreter request forms are received for that class and if an interpreter is available. ADS must receive the interpreter request form 12-24 hours in advance, but not more than 24 hours in advance of each time the class meets.
IMPORTANT: To cancel services for semester courses, a Interpreter Request Cancellation form must be completed as soon as possible.
