Formstack forms are centrally managed by the Harper Web Team. They can be embedded in any Harper web page using embed code, and are ideal for ongoing, business-crtiical use cases. An example of an embedded form can be found on this page.
Formstack is used for ongoing, business-critical forms. Forms that are used for a one-time event, short survey, or to collect data for any limited use-case should be made using Microsoft Forms. Any Harper employee can create a Microsoft Form in OneDrive. If you are not sure, consult the Web Team by submitting a Web Request Form.
Formstack licenses are needed only for users who regularly create or make ongoing updates to their forms -- most forms only require occasional updates. If you only need to receive the form submissions, the Web Team can create a form for you, and you can receive notifications every time a submission is received or batch reports at regular intervals.
While Formstack does offer a number of security features, no system is completely safe from hackers. Please consider that any information you collect could potentially be accessed by unauthorized individuals.
For that reason, forms should generally collect the minimum amount of information necessary to accomplish a specific business goal that also benefits the user. Consider leaving out questions about users' birthdate/age or gender, for instance, unless that information will be used to directly improve the users' experience at Harper at this point in their educational journey.
Note: If you need to collect medical information, social security numbers or other sensitive information, enhanced security features will need to be enabled on your form. These features will limit some of Formstack's functionality.
The best way to protect your users' privacy is to ask for as little information as possible.
The form below shows the different kinds of fields available in Formstack. Additionally, fields can be displayed using conditional logic. To see an example of conditional logic choose Option 2 from the Dropdown Box. An additional Long Answer Box should appear. An asterisk (*) indicates a required field (see the Short Answer field as an example).
